Most of the personal information I hold is provided to us directly by you to ensure it is safe to proceed with treatment
I may also receive personal information indirectly, from the following sources in the following scenarios:

If you have been referred by another medical practitioner or therapist, there may have been information passed on to me by that therapist.
I use the information that you have given us in order to:

Create the best treatment plan for the individual

Ensure it is safe and appropriate to proceed with treatment

Make a referral to another healthcare practitioner if deemed necessary by both therapist and client.

How I store your personal information:
Your information is securely stored.
I will keep your case history information and appointment records secure in a locked cabinet, these records will be kept for seven years following the last date of treatment given. Or in the case of children under 18, records are kept until their 25th birthday. After this time, I will securely dispose of your information.

Under data protection law, you have rights including:
Your right of access - You have the right to ask me for copies of your personal information.
Your right to rectification - You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.

Your right to restriction of processing - You have the right to ask me to restrict the processing of your personal information in certain circumstances.
Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that I transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
